Applying For Assistance Print E-mail
To apply for Energy assistance you will need to contact the local office in your county. If you have any Questions about what you read on this page call your local office. You can find these numbers on the contact page.

Below is everything we need. If you don't bring everything your appointment it can't be started.


  • Gas Bill
  • Propane invoice

 

  • Electric Bill (if you don’t have one get from your energy provider, must contain service address on bill along with actual billing name. Utility bills containing a stub with no information are not accepted)

 

Final Utility Termination Notice (if you've received a Disconnect notice from your utility company).

 

  • If your utilities are included in your rent then you must provide a lease.

 

  • Social Security cards for all persons living in your household. See notes on below

 

  Here is a list of forms of income that must be reported.

 

  • Wages and Salaries For all in the home 18 or older. 1 Full year (Must have at least 13wks of pay stubs.) Also (If household member is 18 and in SCHOOL a current document showing active status must be shown from school.) Anyone 18 or older with 0 Income** must get a Wage Inquiry (must be date stamped), from the employment office, and sign zero income form here in our office.

 

  • Unemployment Compensation Benefits (Bring all stubs received or get a Voucher inquiry from local unemployment office.) see back for address and directions.

o   Self-employment Wages (1040 plus a schedule)

o   Temporary assistance for needy families (TANF), (Printout for 12 months from case worker)

o   Child Support (Then you must obtain a Child Support print out for 1yr, or bring in checks)

o   Social Security Benefits - (SSI,SSDI,SSD,SSA)

o   V A Pensions , or Veterans Disability

 

  • Pension Funds

o   Worker’s Compensation

o   Maintenance (alimony) & Interest and Royalties

o   Insurance Benefits and Annuities Severance Pay

o   Income from Estates and Trusts Training Stipends

o   Rental Income, (if you’re a landlord only)

o   Strike Benefits

o   Black Lung Payments


Some Forms of Income Verification we Use, this may not cover every type of verification you can use.

 

ü  Bank statements can be used for Social Security benefits*, Veterans benefits etc… NOTE: if you are using a bank statement the actual deposit must be identified along with it being an official statement sent from the bank through the mail.

 

ü  Award letters for Social Security can be used; they must state what type of benefit you receive.

 

ü  Pensions checks, Veterans Benefits etc…

 

ü  Verification of earnings from employers, checks, itemized reports.

 

ü  W2’s received (Only until April 15th current year)

 

ü  All Pay Checks received weekly, bi-weekly, monthly, and semi-monthly

 

ü  Child Support print outs for 12 months (if you have received some during the year but not all 12 months a full year must be shown)

 

ü  TANF Printouts form (IQCH) for 12 months (if you have received some during the year but not all 12 months a full year must be shown)

 

ü  Court papers can be used to show child support or maintenance payments, also to prove separation or divorce.

 

ü  Our Forms to verify your income with your employer.

 


 

Here is some information that maybe useful to your family. 

 

Ø  If you’re married but separated; proof the spouse is out of the home is required. (Court Papers, Letter from DFR, Letter from Trustee are some types of proof, and proof of permanent address) If no paperwork can be shown all income must be counted for each adult.

 

Ø  If you use a bank statement; nothing can be blacked out. Official Bank statements are only accepted; no letters or transaction details or screen shots printed by bank can be used.

 

Ø  If someone in the home is 18 years or older in high school; (not adult school) then a current progress report showing active status must be shown for us not to count their income.

 

Ø  If you receive a form of a Social Security or pension; an Award letter can be used, but it must state what type of benefit you receive, or a Bank Statement for previous month can be used. (If your income is direct deposited in your bank.) *It must show what type of Social Security is deposited. If it doesn’t show you can get this information from the social security office.

 

Ø  If you have a college student in your home; and your counting them in your home. You would need to provide all income for 12 months. If you choose not to count them in your home you will need to bring in current tax forms showing you claimed them as a dependant for the current tax year, along with a college schedule showing 12 or more credit hours.

 

Ø  If you have been terminated; a termination letter from your employer will be needed along with proof you filed for unemployment with letter they sent you called a Wage Summary and Calculation sheet. (Only need termination letter if your family is over income guidelines and we are using drastic loss.)

 

If you have been subject to layoff / plant closings; proof of this will be required.

Ø  Social Security Cards; Social security cards for all members are required or a document from the social security office showing the number on page 2. No other forms can be used to verify the number. (HSI can’t and does not pull from previous year files) Any altered cards are not accepted, erased or handwritten numbers on card are not allowed, any erased areas on the card,(name, numbers) void card and will not be accepted, if you have a section that can’t be read it will not be accepted.

**If you have someone in the household with Zero Income; you must bring everyone over 18 who hasn't worked all 12 months, everyone with zero income must sign our zero income form. No one can sign our form for them; unless it’s a POA (paper work will be required). We don’t send the form out of the office. The appointment will not be started unless everyone and everythign is present.
