 Child Care Development Fund Child care Development Fund (CCDF) documents are primarily used for recertification, and/or updating information on a pre-exisiting CCDF case, You may also download and obtain a pre-application to get onto the CCDF waitlist.

Please use the provider information page to obtain the providers rates for child care. You must bring this to your appointment. This can only be filled out by an approved CCDF provider. You may obtain a list of CCDF approved providers by calling 1-866-693-0672
When you download this pre-application, you (the applicant) will need to complete the pre-application in its entirety. In addition to you must provide proof of current service need. Some types of proof include: - Current School Schedule
- Most recent paystub
- Or both if applicable
If you are on TANF impact, we will need a referral from your caseworker. Doing this will allow you to bypass the waitlist process. Make sure you print clearly and neatly, we need all names and DOBs for everyone in the family. This includes mother and father if applicable, all children in the household. If you have a two parent household, both parents have to show proof of a current service need and must meet CCDF income guidelines. If your application does not include all information or does not include proof of service need, the application will not be accepted. This could cause a delay in your intake process, if you have questions please contact your local intake agent.
You may now choose to complete your Child Care Development Fund (CCDF) recertification by mail instead of attending your usual recertification appointment. Do not download this recertifcation if you are not currently on the CCDF program.
